In early 2025, as part of the "Working with Families Involved with Child Welfare Services" Tailoring Services Package, Parents as Teachers released an adaptation to the Child Health Review to assist affiliates in meeting the needs of all families being served.
Unlike the Child Health Record where one is completed for each child, a single Adapted Child Health Review Record is completed with the parents for all children in out of home placement and can be found under both the family assessment and child screening sections in Visit Tracker for your convenience. Please review the Adapted Children's Health Review Record Instructions document found in the Working with Families Involved with Child Welfare Services section on the Tailoring Services page of the Model Implementation Workspace for guidance on using this record with families.
