Step-by-step instructions on creating a new family referral - CIAT - and submitting to the Coordinated Intake Agency.
IMPORTANT - DO NOT CLICK ON "NEW FAMILY". YOU MUST BE IN THE "CIAT" SECTION OF VISIT TRACKER
STEP 1 - Click on CIAT in the navigation bar
Now you are in the Coordinated Intake section of your Visit Tracker account.
STEP 2 - Click on NEW CIAT
STEP 3 - Begin filling out the CIAT
All fields at the top of the form are required. The phone number OR email address have to be filled in.
"Have this referral returned to your agency" - KEEP THIS FAMILY
- Check this box in order to indicate to Coordinated Intake that you want this family referred back to your agency
- When this box is checked, the return process can be automated. This feature has to be turned on by the Coordinated Intake Agency that is receiving and processing this family CIAT.
"Additional family data"
- are optional fields.
- NOTE: Provide as much detail as possible for the Coordinated Intake Agency.
STEP 4 - Click the "Save and Send" button to submit to the Coordinated Intake Agency your account is connected to.
