Referrals related to screenings are recorded directly on the screening record.
There are 2 questions in the Resource Connection section of the screening record.
"Is the home visitor providing support related to a concern found during this screening?"
If the home visitor will be providing additional support for the family due to a concern found in the screening, select YES for this question.
If no other referral is being made, that is all that is needed to indicate the family is receiving additional support.
"Was a resource connection made as a result of a concern found during this screening?"
If a referral is being to an outside agency, then choose YES for this question. The Resource Connection Record will open up and referral details can be entered along with follow-up information.

Open the record, update the status and/or add a note.
